P1 - PSMA-guided surgery

In recent years imaging of prostate cancer lesions has evolved in an almost revolutionizing way - especially by the use of positron emission tomography (PET) imaging with ligands directed against the prostate specific membrane antigen (PSMA) , a type II transmembrane glycoprotein with overexpression on most prostate cancer cells. By chemical modification and labelling of PSMA tracers with gamma-emitting radiopharmaceuticals also SPECT imaging as well as intraoperative guidance could be achieved (a procedure termed “PSMA radioguided surgery”). In PSMA radioguided surgery , the trapped radiation within cancerous tissue can be detected in vivo using a gamma probe and successful resection can be immediately confirmed ex vivo.

Nevertheless, several clinical and preclinical questions on the use of this novel technology need to be addressed, among them the preoperative identification of most suitable patients, the extent of surgical dissection as well as the identification of novel predictive biomarkers. In this context, two investigator-initiated prospective trials are recruiting currently ( NCT 04271579 ; NCT 04324983 ). Furthermore, refinement of the surgical approach using novel gamma-probes for robotic minimal-invasive surgery are currently investigated. Close collaborations with the Department of Nuclear Medicine , the Institute of Tumor Biology , the Institute of Pathology , the Institute of Radiation Biology among other partners exist.
