If you would like to take part in the questionnaire study, please register using the form below. Please also indicate whether you want to participate in the additional telephone interview.

If you are interested in taking part in a telephone interview (approx. 30 minutes), please register using the form below. We will contact you by email to arrange a suitable time.

- send you links to the questionnaires
- contact you regarding the interview (if applicable)
- send you a voucher (if applicable)
Your email address will be used exclusively for these purposes and will be deleted after the study has been completed. Your questionnaire responses will be stored anonymously and processed separately from your contact details, so that no conclusions can be drawn about your identity.
